Here in NM, the Donate Life plate is available for a ONE-time fee of only $12.00!
Other states that offer them (fees vary) include: Maryland, Virginia, South Carolina, Connecticut, New York, New Jersey, Pennsylvania, West Virginia, Ohio, Wisconsin, and Arizona. There may be others also; this is what I found with a quick Google search.

No delayed gratification for me -- I wanted one RIGHT AWAY! I just sent in a form (downloaded off the Internet) and a check and got the plate in the mail about a week later. Super quick and easy. May be different in other states, though.

Cool, I'm going to check if Maryland offers them. My tags are due in February and I want one of those.

How awesome!! I just sent the transportation cabinet an email to request one for KY. They have every other cause under the sun, so they better add this one!

We seem to have every other "cause" but nothing for organ donation.
Specialized License Plates - CALIFORNIA
Special interest license plates have a background image and help fund a specific cause:
Environmental: environmental programs (only available personalized)
Memorial: scholarships for children of 9/11 victims
Arts Council: art programs
Whale Tail: California Coastal Commission
Collegiate: need-based scholarships and grants
Lake Tahoe Conservancy: lake conservation programs
Yosemite Foundation: park wildlife and habitat fund
Firefighters: firefighter memorial programs
Help Our Kids: child health and safety programs
Olympics: training center in San Diego
Veterans Organization: county veteran's services
There are at least 13 more plates available, including Pearl Harbor Survivor, Horseless Carriage, Antique Motorcycle, and others. For these, fill out a Special License Plate Application.

I checked with the State of Maryland and this tag is offered.
I'll have mine in February. I just think this is such a good idea for getting the word out. How many people don't read different license tags.
