First time arm swelled and is very sore,should I be concerned?

They have been using one needle on my fistula and been placing it in the upward position. Yesterday the tech put it a little higher on my arm and put it in the downward position.He said they had to start using the whole fistula to get it used to being used. That makes sense. It worked fine this time but in the early afternoon I noticed the arm was somewhat swollen and was very sore. This morning it is still swollen and still very sore when I move it.
Is this common when the needle is first placed in the higher position on the arm or should I be concerned? There is no brusing so I know I have not bled out at the site.
I never hurt nor did I swell all the other times .My fistula is very close to the skin so I don't have to use any type cream to numb it as I don't have any real pain when they stick me. That is why I don't understand the swelling and discomfort I feel.

Another `happy' thing happened yesterday,they switched our times without even contacting us first. I was going in at 9am unless they had a chair earlier. NOW I have to be there at 5:30 am and on by 5:45. Is this a common practice at other clinics?
