Antibiotics for tooth extraction

All of my mom's tooth has to come out. The dentist is planning on taking one quadrant out. When I asked him about pre-op procedures, he said he will give her antibiotic (penicillin) just right before the surgery and then some for afterwards. Would that be ok (or) should she be put on antibiotics way ahead before the surgery? And, should local anesthetic or gas be used for the procedure? Her dialysis is on the following day after the procedure. Please advice.

We are not doctors on here.  We also do not know your mother's medical history.  Plus, not everyone presents the same.  These are important questions that you should be asking of her nephrologist.

All the neph told me was to follow the dentist's advice and that penicillin is safe. But he is not sure if it needs to be taken in advance or just right before the surgery. Thanks.

The neph didn't offer to find out for you?

Since your mother may still be prone to bleeding the day after extractions, you might want to ask her nephrologist if the amount of heparin normally given during treatment will need to be modified and if she should get saline flushes as well to keep the dialysis filter from clotting.

EDIT:  If your mother experiences significant bleeding after the extractions, let the dialysis center charge nurse know so they can check her hemoglobin.  If low, they can give her EPO, Aranesp, or whatever red blood cell production med used at her clinic.

Thank you. Nope, that is all he told me. I know that sounds scary but that is how the treatment and care goes here, and we do not have any other way of resolving this. Now, I know what exactly to ask him! Thanks again!

Thanks for letting me know. The dentist said my mom will get antibiotics just right before her surgery. I do not know what he meant by that and if that would really help rather than giving a couple of hours earlier like you mentioned!

4x500mg Amoxicillin, I assume.

I think the massive dose of antibiotic as a preventitive could work well.

I myself hate that as in me this tends to kill off the intestinal flora leading to a day or two of diarrhea.  I hate that.   Funny how the same AB given IV doesn't do that.  I think it is simply the massive amount that must be ingested so to get enough absorbed through the intestines to get an adequate blood concentration, where an IV doesn't need near as much and it doesn't readily pass from the blood into the intestine.

It's been a long time since I've been to the oral Surgeon but mine used an IV.  He would gas me initially, I think that was just to relax me a little, but the actual knock-out stuff came in the IV.   All I know is it was lights out!    And then I woke up, done.

Yep, since it takes about a hour to get to my dentist just as I go out the door. I currently live in New Jersey and the traffic is horrendous.
