Patients and Caregivers: NKF Dialysis survey - $ paid, if you qualify

Received an email today:

Dear Friend,
The National Kidney Foundation (NKF) is conducting a survey among people on dialysis and their caregivers to learn more about their experiences and thoughts. Findings from the survey will help us do a better job in providing the needed education and support for all patients on dialysis.
The survey asks about the current dialysis treatment. There are 50 questions. It will take approximately 30 minutes to complete. If you qualify to participate in the survey, and complete all the questions, you will receive a $40 (cash value) bank card as a thank you gift.

we both did this, just received 2 american express gift cards, tried to use them yesterday at merchants, both were declined.

card doesn't say need to be activated, no number to call for info.

I got my gift card also but have not tried to use it yet. Not a good thing if they don't work.
