I believe I started out on 2mg of tac twice a day. I'm now two years out and have been at 1.5 mg, twice a day, for over a year. You start out on so many drugs and it takes pretty much a year before things settle down. The Prograf is not what they fiddled with so much for me. Rather at one point they took me off Myfortic completely. That is not a drug that has a pleasant re-introduction, in my experience.

So you only take Prograf and nothing else?? That can't be right correct?

What about prograf in the beginning?  You HAD to have been on a higher prograf dose at the start. The guidelines call for .2 mg per kg of bodyweight per day! That means anywhere from 10 mg to 40 mg of prograf a day to start!

You can't really go by "dose" becasue we all absorb it differently. A number of years after both my friend and I had our transplants, we both had a target prograf level of between about 6 and 8. And I was taking 1.5 mg twice per day and she was taking 7 mg in the am and 6 mg in the pm. And I outweighed her by about 50 pounds. It's just how we absorbed it differently.

The bottom line is that once you get to be about a year or so out, they will set a level they want your prograf to be and tend to stick with that.

I started at 4 mg twice a day with a target trough range between 10 and 15. I'm on 2 mg twice a day now and my trough level is at 6. I'm 2 years post transplant.

Hey DGF I also take 360 mg of Myfortic twice a day besides the 1.5 mg of tac twice a day. That's my transplant meds. The only other med I take is 10mg of avorstatin. I searched through my records and I was on 4 mg tac twice a day right after my transplant as well as other meds. They went down, back up and then down again on the dosage. I weighed under a hundred pounds after my transplant and am about 105 now. I don't know about the tac trough. I only became aware that through IHD but didn't ask about it or notice it on any of the lab reports.

My husband is 9 months out from his transplant, he takes 2 mg of Prograf in the am and 1 mg in the pm. He also takes Cellcept and prednisone for the transplant, along with all his other meds. In all, he takes over 25 pills a day.

A better question is what was your prograf blood level the first month or 2 and what is it after that?

They say the first 2 months your prograf blood level should be higher.
