Kt/V has been the primary measure of dialysis received for almost 40 years, but has come under significant criticism for not only its limitations but also in the way providers in some parts of the world manipulate its use at the expense of patient outcomes. The disturbing trend of decreasing time and increasing pump speed to maintain volume is being driven mainly from the US, which is seemingly ignoring advancements in understanding of appropriate dialysis dose.
