just waiting to see whats next

well looks like the neph was wrong since aug of last year by GFR has dropped from a 45 to a 17 by BUN is 61 ...  creatinine 3.87 so my question is whats next  I had a long summer trip planned with my daughter now I'm unsure if I will be able too go. I would appreciate any opinions

I have scheduled an appt. with My Neph but would like to hear when other people started dialysis also I am a type 1 diabetic

Well, next would be and access.  Either a fistula for hemodialysis or a abdominal catheter for PD (Peritoneal Dialysis).  Once you get your access it is a waiting game.  My experience is that they start dialysis when GFR gets to 10.  But everyone is different.  Ask his/her advice on the summer trip.  I would like to see you go, but if you are going to be sick the whole time it would be no fun for your or your daughter.

My experience is that they start dialysis when GFR gets to 10.
The general rule of thumb is start at gfr of 10, unless diabetic, then start at 15.  If starting PD, do it sooner to preserve residual.

The actual decision is a complex medical judgement and varies with the individual, which is why nephs get to charge what they do.

If I did PD I could still travel couldn't I ? I'm looking to try and do home treatments this trip is very important to me As I don't see my health getting any better.

You can travel on PD. You can also travel on hemo. Both take some planning ahead. If it's a really popular place, you have to schedule hemo on vacation far in advance. Either way, I wouldn't recommend traveling if you've only been on dialysis a few weeks. Your body needs some time to adjust to dialysis. You need time to feel familiar with your treatment so you know if something is wrong. Definitely ask your doc what he sees as the timeline and treatment for you.
