We got our monthly nephrologist visit yesterday.  I had good clearance news - 93%!  But some bad news.  My PTH is 810.  He will proscribe Sensipar, but says he has to wait until I bring up my calcium.
I have not been good with my binders - Goofynina would be very annoyed with me!

Dumb question.  If you need to bring up your calcium, why doesn't he prescribe Calcitriol?  Two birds with one stone - more calcium and lowers PTH.  That's what they do with me.

I won't be able to ask him that until next month!!  I will check with our unit supervisor and maybe through emails she could get him to let another doctor prescribe that.

bajanne...take your binders!!!  Don't make me have to come down there.  On t'aime trop pour accepter que tu te négliges de cette façon.  Remember food and binders go together like a horse and carriage....you can't have one without the other.  :flower;

Uldall-Cook catheter inserted May 2008
Haemo-dialysis, self care unit June 2008
(2 1/2 hours X 5 weekly)
Self-cannulated, 15 gauge blunts, buttonholes.
Living donor transplant (sister-in law Kathy) Feb. 2009
First failed kidney transplant removed Apr.  2009
Second trx doing great so far...all lab values in normal ranges

Diagnosed Nov 2007 with Multiple Myeloma.
By Jan 2008 was in end stage renal failure and on haemodialysis.
Changed to CAPD in April 2008.  Now on PD with a cycler.  Working very part time - teaching music.  Love it.  Husband is Paul (we're both 46), daughter Molly is 13.

VEEERY important to control the phosphate with binders.

I agree about calcitricol with the calcium. I have been on that for years - indeed mine is going a bit high so we might cut back a bit.

Sensipar though is a wonder of a drug. It works perfectly for me. Indeed the other month my PTH went down a little too much so we had to cut back on the sensipar. Good stuff that!

93% is bloody awesome clearance!! I wish I could get close to that (my last was 78%) - I am envious!

Well, I am running scared and since my dialysis on Friday, no one is more compliant about binders than I am! every meal, snack, bit, bite!!  [hope i can keep it up!]

What type of binder do you use?
Calcium based binders such as PhosLo or Tums?

Or are you using Renegel?

Also, as peleroja said, adding Calcitriol --which is active Vitamin D-- helps you absorb more dietary calcium in the gut and it also suppresses the parathyroid.

Or better yet, why not receive IV Zemplar, Hectoral or Calcijex (all active Vitamin D) during dialysis, since IV solutions are more effective than oral medication.

Fresenius 2008T dialysis machine
My KDOQI Nutrition (+/ -):  2,450 Calories, 84 grams Protein/day.

I am supposed to get Zemplar during treatment.  Haven't heard them tell that they gave me recently. (but then sometimes I am asleep when they give me)

I am using Tums.  PhosLo is the recommended binder in our unit, but it is rather expensive for me these days.

A friend in my unit gave me a little snack bag yesterday at dialysis - it was full of PhosLo!

And you guys will be very glad to know that I have been taking my binders consistently!!  Yes, Goofynina, finally!

MY PTH is increasing gradually every month. Darn it!  Phos and calcium are normal. I take 2 recalcitrol in the morning. Any ideas?
