No transplant here, just dialysis but my husband was on the highest amount of BP meds pre dialysis. And back then even with those meds 150 over 110 was low for him.  Within two months of dialysis he went off everything and he has had perfect BP since then. Same thing 20 years of high BP fixed just like that.

I was on 4 BP meds pre-dialyis.   Now I'm on a beta blocker (tiny dose, for tachycardia, not hypertension) and rarely break 100 systolic.
