Creatinine Level while on dialysis

I started dialysis 5 years ago with a creatinine level of 5.5. Its now been as high as 17-18. Im on the PD cycler at nigh. I am only 4'11 and 108lbs. Is there a reason why its this high on dialysis?
My kt/vis within range, and has always been.

Creatinine of 5.5 is low to start dialysis, it would put you in stage 3 kidney failure.  But creatinine is not the only deciding factor, it also depends on your glomerular filtration rate among other things.
