In-center Nocturnal Hemodialysis May Reduce Mortality Risk

ORLANDO, Fla.—Patients who undergo in-center nocturnal hemodialysis (INHD) have significantly better two-year survival rates than well-matched patients receiving convention hemodialysis (CHD), researchers reported here at the National Kidney Foundation's 2010 Spring Clinical Meetings.

INHD provides a promising alternative dialysis modality that sits between thrice-weekly in-center CHD and nocturnal home hemodialysis, they concluded.

Eduardo Lacson, Jr., MD, MPH, of Fresenius Medical Care North America, Waltham, Mass., and colleagues, compared 771 INHD patients with 2,045 CHD patients. The investigators matched patients by propensity score (including age, gender, race, presence or absence of diabetes, body mass index [BMI], dialysis vintage, and other variables), facility location, and incident/prevalent status (on dialysis for less than or equal to 90 days vs. more than 90 days).

INHD patients had a 19% reduction in two-year death risk compared with CHD patients after adjusting for age, dialysis vintage, and BMI. The authors acknowledged that the study was observational and residual confounding may still be present.
