Community Nutrition Survey for Renal Patients

I am a Nutrition student at LaSalle University in Philadelphia, PA and interested in becoming a Renal Dietitian.

I took the survey, but I don't know how the results are going to come out. A few of the questions had confusing answer boxes. I wasn't sure how to answer, so I just winged it.
