Title: National Kidney Foundation & Columbia University Renal Epidemiology Survey

The National Kidney Foundation is working with the Columbia University Renal Epidemiology (CURE) Research Group would like to invite you to complete a quick survey to help them better understand what is important to kidney patients who are either seeking a kidney transplant or who have received a kidney transplant in the past.

This survey asks questions about what is important to you in choosing a transplant center (a hospital that performs kidney transplants), as well as about your understanding of the kidney transplant process. As kidney patients, it is very important that we hear from you as your feedback will help us to improve the kidney transplant process and ultimately make things better for kidney patients. This survey is intended for patients - any caregivers who take this survey should respond on behalf of the patient. If you are willing to participate, please know that we are deeply grateful for your time and contributions.

This survey is completely anonymous and will take about 10 minutes to complete.
