Although the number of dialysis centres is increasing in many regions of Pakistan, most patients stop treatment within the first three months because of cost constraints. Dialysis is not a permanent solution. It is rather an interim arrangement to keep alive the patient whose kidneys stop functioning.

Kidney transplantation is another option, but only about five per cent patients receive a transplant. The shortage of donors is a universal problem, and paid organ donation accounts for 70 per cent of all transplantations in Pakistan.

Most kidney diseases attack the tiny units inside the kidneys called nephrons, causing them to lose their filtering capacity. Damage to the nephrons can happen quickly, but most kidney diseases destroy them slowly and silently. Kidneys have to separate urea, mineral salts, toxins, and other waste products from the blood. The actual removal of wastes occurs in nephrons. Each kidney has about a million nephrons.

Kidneys are located nearly at the bottom of the rib-cage in the right and left sides of the back. The left kidney lies a little higher than the right kidney. Kidneys are bean-shaped, about four inches long and two and a half inches wide. Although the body is equipped with two kidneys, you can function with one reasonably healthy kidney if the other is damaged or removed.

Some people are born with only one kidney but can still lead normal, healthy lives. Every year, thousands of people donate one of their kidneys for transplantation to members of their families or friends.

High blood pressure plays a major role in the rapid progression of kidney problems in people with diabetes. High blood pressure can damage the small blood vessels in the kidneys. The damaged vessels are thus unable to filter wastes from the blood properly.

People whose kidney disease becomes worse may need to urinate more often or less often, feel tired, lose their appetite or experience nausea and vomiting, have swelling in their hands or feet, feel itchy or numb, get drowsy or have trouble concentrating, have darkened skin and have muscle cramps. Serious health problems occur when people have less than 25 per cent of their kidney function.
