Have you had to have any drugs for other reasons?  Like chemo or steroids?  Sometimes those drugs can do strange things to your hair.

Make an appointment with a dermatologist and see what they have to say.  The pimples are not anything I have experienced.  The hair loss, yes.  I think the stress of starting dialysis made me lose my hair.

Try Paul Michels Tea tree shampoo for the head acne.  It worked for me.   Have you had any fevers that lasted a few days or any surgery?  Both fever and anesthetics will make some of your hair fall out about 6 weeks post .

I lost hair while on dialysis. They gave me a vitamin and it seemed to slow down the hair loss.
