Last night hubby hooked up to the dialysis machine as usual for 7 hrs.  I am in charge of setting up the machine and he does the needles. When he had about 6 hours done we got TMP alarms and then arterial and venous alarms. He had clotted!!!  Just got back the blood through the arterial needle rest had to be thrown away.  Good job his hemoglobin is always a bit on the high side!!I hooked up the heparin, primed it but forgot to put in the amount to be infused every hour and how many hours it had to be on. Turned on heparin when he was hooked up but of course there was nothing there to tell it what to do!!  Didn't get an alarm or anything.  From now on we will go back into the heparin screen after he is hooked to make sure that is done.  I felt so bad.  At least he got 6 hrs of treatment!!!  He was good about it though and didn't even get mad with me!!!
