I did not fill the form 2728  after transplant Since I read  from the internet and confirmed by the esrdnetwork people  that no new cms 2728 is needed for the transplant patient switching from dialysis. Now I just find out that ssa requires the transplant patient to report the transplant ,so they will decide terminating your benefit or not after 36 months. Could not sleep now.  Do not know what to do.

Will call them. You had dialysis before transplant? If so make sure the saa knows you had transplant.  Thanks
