Announcement from ProPublica:

Today, ProPublica launches our new patient safety section - http://www.propublica.org/article/how-you-can-help-propublica-investigate-health-care-quality - where readers and providers can be part of the patient safety conversation, get regular updates and share stories or views.  The section will include links to many of our healthcare investigations, questionnaires for patients and healthcare providers, information on our Facebook patient harm group - https://www.facebook.com/groups/patientharm/ - (where nearly 1,000 people share their personal stories of medical mistakes) and more.

The section also has:
