I would guess there is someone in charge of him...If not mmm medical board? Insurance?  What is he or she doing?

Make sure you really want to first.  If it is something that happened last night or yesterday, wait until tomorrow.  Then write HR, Corporate Headquartes and CC him.  But document .. specific somplaints and not attitude stuff.  When and under what circumstances. etc.

Come to think of it, nothing is funny anymore.

Nothing that I post here is intended for fact but rather for exploration into my personal thought processes.  Any slight, use of words with multiple connotations or other percieved insults are totally unintended.  I reserve my insults for private.

No one likes having you go over their head!

One of our potential donors challenged the head of transplant over being rejected (not for medical reasons) and the s**t hit the fan.

So, be very careful in how you approach this.

My unit is located at a UCSD hospital. They take any complaints extremely seriously!! Complaints are thoroughly investigated and are reviewed by a board. I don't know all the particulars but I do know registering a complaint is not discouraged and taken extremely seriously. We (patients) are very lucky to have this policy in place.
