These are sentences actually typed by medical secretaries !!!!
1) The patient has left her white blood cells at another hospital .
2) She has no rigors or shaking but her husband states she was very hot in bed last night .
3) Patient has chest pain if she lies on her side for over a year
4) On the second day the knee was better and on the third it disappeared.
5) The patient has been depressed since she started seeing me in 1993
6) Discharge Status ..Alive but without my permission
7) Patient had waffles for breakfast and anorexia for lunch
8)While in ER she was examined ..x-rated and sent home.
9) The skin was moist and dry
10) Patient was alert and unresponsive
11) She stated she was constipated for most of her life until she got divorced
12) Examination of the genitalia reveals he is circus sized
13) The lab tests indicated abnormal lover function
14) Skin , somewhat pale , but present
15) Patient has 2 teenage children but no other abnormalities
Told you , you need to stay away from hospitals !!!
