Here in NM, the Donate Life plate is available for a ONE-time fee of only $12.00! 

Other states that offer them (fees vary) include:  Maryland, Virginia, South Carolina, Connecticut, New York, New Jersey, Pennsylvania, West Virginia, Ohio, Wisconsin, and Arizona.  There may be others also; this is what I found with a quick Google search.

Pyelonephritis (began at 8 mos old)
Home haemo 1980-1985 (self-cannulated with 15 gauge sharps)
Cadaveric transplant 1985
New upper-arm fistula April 2008
Uldall-Cook catheter inserted May 2008
Haemo-dialysis, self care unit June 2008
(2 1/2 hours X 5 weekly)
Self-cannulated, 15 gauge blunts, buttonholes.
Living donor transplant (sister-in law Kathy) Feb. 2009
First failed kidney transplant removed Apr.  2009
Second trx doing great so far...all lab values in normal ranges

1993 diagnosed with glomerulonephritis.
Oct 41, 2007 - Got fistula placed.
Feb 13, 2008 - Activated on "the list".
Nov 5, 2008 - Received living donor transplant from my sister-in-law, Etta.
Nov 5, 2011 - THREE YEARS POST TRANSPLANT!  :D

GREAT.  I will see about getting one here in Virginia the next time we renew. 
No delayed gratification for me -- I wanted one RIGHT AWAY!  I just sent in a form (downloaded off the Internet) and a check and got the plate in the mail about a week later.  Super quick and easy.  May be different in other states, though.

We seem to have every other "cause" but nothing for organ donation.

Specialized License Plates - CALIFORNIA
Special interest license plates have a background image and help fund a specific cause:

Environmental: environmental programs (only available personalized)
Memorial: scholarships for children of 9/11 victims
Arts Council: art programs
Whale Tail: California Coastal Commission
Collegiate: need-based scholarships and grants
Lake Tahoe Conservancy: lake conservation programs
Yosemite Foundation: park wildlife and habitat fund
Firefighters: firefighter memorial programs
Help Our Kids: child health and safety programs
Olympics: training center in San Diego
Veterans Organization: county veteran's services
There are at least 13 more plates available, including Pearl Harbor Survivor, Horseless Carriage, Antique Motorcycle, and others. For these, fill out a Special License Plate Application.
