WOW, Congratulations  :2thumbsup;  As far as the tape goes, it sounds like you are allergic to it, you might want to try a cloth tape called Medipore,  the same thing happened to me (the tape, not the pregnancy) lol, but the Medipore seems to be working well for me, and as far as "how it keeps happening"  well, there is this little bird and there is a little bee... ;)  Good luck to you with everything  :2thumbsup;

I sure second the recommendation for Medipor tape, it's made by 3M and Sharon needs it for the permanent dressing on her back (changed weekly, but will always be there) no adverse reaction like so many others.....

I had to start using different tape when I started doing button holes. It's called Mepitac and is great, no marks on my arm at all and no itching.

Are these other tapes available at regular drug stores, or do you get them online or through a  specialty pharmacy? I was given a sample of Hypafix (I think) and it's not bad, but it's pricey and I have to figure out where to get it.

There are 3 main tapes that are used at my unit.  I am allergic to the paper-type tape - it itches badly.  Then there are a plastic-type and a fabric-type ones.  The nurses tend to ask you each time which one is okay for you, because it seems that different types affect different people.
