I was not required to get any dental documentation, and my rectal probe was up2date, however, I was required to visit an infection disease specialist. No, she didn't check to see if I was infected - it was a review of all my vaccinations. We weren't sure if I had a measles vaccine, so I got that one done (already had the shingles vaccine, which I had requested for the reason you gave). I also noticed my primary MD is more aggressive about vaccinating me because of my ESRD - I am below the age at which the pneumonia vaccine is recommended, but he pushed me to get it (which, interestingly enough, was offered by my dialysis clinic at a later appointment).
If you are over 50, you may want to consider a shingles vax since you will be unable to get it after tx (it's a live vax).
You might want to ask you MD about this vaccine if you are a transplant candidate under 50 as well.
