ESRD Facility Transition and Low volume

Hello,

I am new to this board and was hoping for some answers
1. Many of you might know that CMS came up with new ESRD PPS starting CY 2011 where facilities can opt in or out of blended payment transition. Is there a source from where I can find these facilities which have elected to opt in or out. I have looked at CY 2011 and CY 2012 ESRD Impact files and they have facilities that have elected for transition but CMS has put it out in the proposed file and removed it from the final file for 2012 and it says “estimate” in the proposed file.
2. If a facility does not submit its answer to CMS by Nov 01 for transition, CMS will automatically include the facility under transition. Is this correct? If so if a ESRD facility is not in the impact files, does it mean it goes under blended payment?
