graft is spitting out the needles!

As some of you know I had a fistula gram done a few weeks ago and they didn't find anything wrong but ballooned open a spot towards the bottom. Now a few weeks later it's still buzzing away but the astrimosis on the arterial side in that area the thrill is real harsh feeling and the bruit sounds really forceful, it doesn't have a hi pitch just very loud and forceful sounding. In fact we can no longer canulate that area because it's so harsh that the needles pop out of the graft and my pressures bounce all over the place and we get alarms galore.
The upper part of my graft has a nice soft and smooth thrill and a nice smooth bruit while the bottom towards the hand just sounds and feels like it's about ready to blow out of my skin. When we stick the upper arm everything runs normal. Could this be a sign of more narrowing? or what could this mean?

Yes it was near the astimosis on the artial end so we just stick above that area which still gives us plent of room as my graft goes from my elbow to under my armpit.
