Questions about switch from Incenter Hemo to Home PD

I was doing 3 day a week for 4:15 and its destroying me, I got them to switch to 4 days a week for 3:15 and my body seemed to handle it better.  However my doc & the clinic could not figure out how to calculate the quality of dialysis so they made me switch back to 3 days a week.  Amazing enough my body still can not handle the 4:15 & I would like to know if switch to Home PD would help.

You are going to be spending more time doing PD than you do on hemo. When I was doing CAPD, I did 4 exchanges a day, every day, with a 3 hour dwell time. I have since transitioned over to CCPD and do my exchanges at night. That said, while I'm dwelling I can pretty much do anything I want, except work out. I really like how I feel on PD and wouldn't change it for anything.

 If you have a care partner why not try home-hemo? I was in-center for about 2 months and then got trained for home hemo. I feel much better now. It does involve doing D 5 days a week for 3 to 4 hours a day.

I was on PD for 3 years until infections hit the PD tubing.  Now I'm back to in center hemo waiting til I can get back to PD.  I was doing 6 exchanges in 24 hours.  4 on the machine 2 manuals.  LOVE it.  I would suggest at least looking into it.  Discuss it with your Dr.   Grumpy

John has done both home therapys (PD & hemo). PD can be a good form of D, but was not the right one for John.  Now home hemo, in my opinion, is the best for his needs.  Either way, doing it at home is the BEST as In-center just doesnt cut it for some.

Therefore, I did not vote, cause it was for Home PD vs In-Center Hemo.  But I would definately vote HOME anything vs In-Center!

The problem with most incenter hemo programs is that they run you too fast and too high. The body's blood system can only empty and refill at a certain rate, trying to make it go through that process faster is impossible. Combine that with the fact that regardless of your weight or age or other indicators, one size does not fit all for dialyzers. I felt terrible on incenter hemo because they ran me at 500 on a 180 dialyzer. I made them switch me to a 160 and that helped but they wouldn't slow me down. When I went on NxStage at home, I went down to 300-350 pump speed and I feel good. My numbers are excellent, Kt/v was 3.3 this month and I have very few food restrictions. The best thing about home hemo is that you can do it on your schedule, mornings, days, evenings and some can even do it while they sleep. It's also more effective at mimicking what native kidneys do.  I've considered PD just for the convenience  of being able to do things while I'm dialyzing but I'm concerned about the glucose issue that some people have. I have a very low tolerance for disturbances in my glucose levels due to reactive hypoglycemia. I also live on the lake and enjoy going for a dip every now and again, which isn't possible on PD. But it's convenience is a plus.
