Do these actually work?  And how well?  Do you notice much of a difference in your labs?

I think it totally depends on a) how diligently you take them, b) which binder you are on for your personal situation and of course what you eat.   I am currently on generic PhosLo.  I've found that they aren't working so well any more but I also wasn't taking them diligently.  So I have clamped down on my diet and taken my binders religously.  It will be interesting to see how my labs turn out this month.  I have a sneaking suspicion I will be forced to go on a different binder that will be big bucks compared to the generic PhosLo, which is $10 for a 90 day supply with my insurance.

I'm on Renagel. I take 2 with my lunch and 4 with my supper.  I notice a big difference in my labs.

Yes binders do work. If the ones your taking aren't, ask your doctor to prescribe another. Phoslo did not work for me nearly as well as Renvela, which is what i take. My labs are fine, just make sure you take them anytime you eat.

If I don't take binders every time I eat something with a significant amount of phosphorus, my phosphorus labs go up.

I have NEVER, EVER takin ANY, not even one single binder in the ENTIRE time that I have been on dialysis..... and that is 8 months doing in-center Hemo and 6 months on CAPD. 

My Phos runs 3.3 -  4.2 (every single month)

my secret is that I do not eat ANY dairy at all!!! not even a sip of milk!!! other than that I eat whatever I want, when I want...... from hotdogs, (which are high is phos) to lunch meat (another one that is high in phos) now, I do not suggest that other people do ANY of this without first checking with their Nephs...... I am just lucky in this aspect of kidney failure!!!!
