Here is a really excellent atlas covering all aspects of hemodialysis access function and malfunction which all renal fellows should read. I looked at this atlas a couple of weeks ago and found it to be really informative.  It is a must read for anyone with any kind of vascular access.

I'm disappointed, I'm blind and use a screen reader, and this doesn't seem to be screen reader friendly as I wanted to read it.

May 13, 2009, went to urgent care with shortness of breath
May 19, 2009, went to doctor for severe nausea
May 20, 2009, admited to hospital for kidney failure
May 20, 2009, started dialysis with a groin cath
May 25, 2009, permacath was placed
august 24, 2009, was suppose to have access placement but instead was admited to hospital for low potassium
august 25, 2009, access placement
January 16, 2010 thrombectomy was done on access
