Well, I'm fairly happy. As of my last labs, all of my numbers are really good except for my phosphorus.

Now that I'm doing 1950 ml per fill, 5 fills a night, for 10 hours total, plus a manual of 1500 in the evening, my Kt/V is up to 2.14. Actually, now that I remember, that was on 1600x6, plus the 1500 manual.

My potassium was right in the middle of the normal range, and my hemoglobin was up past 11 (which is pretty good for me, I'm used to hovering around 9). My protein was at 3.9, just under the goal of 4.0, but not a problem.

The only problem is my phosphorus at about 8. I've never been able to get it below that. I got my Renvela dosage upped yet again - now I'm supposed to take 5 with each meal, plus a 1000 mg Fosrenol.

My fluid status is quite good too, and with it my blood pressure. BP has been staying around 126/84, and rather than my previous norm of carrying too much fluid, my heartrate is constantly up, probably because I'm actually a bit dehydrated. Not enough to give me leg cramps too often, but just enough.

I asked about shortening my dwells to about an hour each, and adding one more, so I'll be doing 1500ml/3hrs + 2000*6/10hrs, which would mean I'd need to order an extra bag for every night. My PD nurse seems to think it won't benefit my dialysis that much, aside from maybe pulling off a little extra phosphorus.

Its swings and roundabouts really , by shortening your dwell time you are just giving it less time to work and pull off toxins , so not much point in adding an extra one and doing less dwell time. High phos ..have you checked your diet ?

If you are a very high transporter, the short dwell time may work. Shortening dwell time and adding one more fill would definitely help to get more fluid out.  The UF is usually peaking at about 1 hour of dwell time.

I'm not a very high transporter, at least not anymore. I was higher before I got peritonitis earlier this year, but I don't know how much - I don't have the lab reports, and it was a different kind of PET test than the one I had here. According to the most recent one, I'm a high-average transporter up to about 2 hours, and a low-average transporter after that.

I think I just started this thread originally because I was happy about how well my dialysis seems to be going now. I'm holding a lot more fluid than I ever thought I would, and I'm not at all uncomfortable. The dialysis I'm getting is making me feel pretty good. I can remember what it was like to be in bad shape.

Since the phosphorus is so high, it is worth to try. If it works, the lab will show it. If not improving, then something else needs to be done. Protein and phosphorus are so closely related, it is hard to gain one and limit another through diet. Maybe, focusing on foods with high quality of protein, that is fish and other animal meat, and limiting plant protein and dairy products may help to maintain protein level and limit phosphorus.
