A help me question

Wow i have a TON to learn.

I will be starting dialysis soon.  And i want to do home dialysis.

Can someone tell me the differnce between
Nocturnal hemo dialysis ( i hear this is the closet to having working kidneys)
and
CCPD.

I have been reading thread after thread but i think im just confusing myself ...

Both are nocturnal. One is hemo and the CCPD is peritoneal dialysis done on a cycler.

CCPD - Continuous Cycling Peritoneal Dialysis = you hook up to a machine with multiple fluid exchanges during the night.

CAPD - Continuous Ambulatory Peritoneal Dialysis = just like CCPD except no machine is involve.  All are done manually with the solutions hanging from an IV pole.  This is what I've been using for almost a year.

Don't know much about Hemo Dialysis, sorry.

Both are done at night, both require a machine.  The difference is what you are using for a "kidney".  In the CCPD, you use a catheter inserted in your abdomen, and pump fluid in and out all night.  Basically, the membranes in your stomach actually do the filtering - the toxins leach into the fluid, and then you drain the fluid and replace it with clean fluid.  Sort of rinse and repeat.

Nocturnal hemo uses a fistula access or a permacath that lets you access your blood directly.  If you have a fistula, you need to insert needles.  The permacath is inserted directly into your artery, and has tubes sticking out that lets you connect to your machine without using needles.  One needle pulls blood out, the other pushes it back in.  The machine is set to clean your blood by running it through an artificial kidney.  Nocturnal works better than regular in-center because it's done longer and slower, which is gentler on the body.

There is lots to learn, indeed!  Keep asking questions until it all starts to make sense!

From what I've read about haemo you'd probably get a better nights sleep using a PD cycler. The advantages are that the machine is only active every 2 or so hours and in between (Dwell Time) it is almost silent and there is a way to make it totaly silent during dwell time. Also you can lie in any position you want whereas with nocturnal haemo I think that you'd have to stay in one position all night in case you disturbed the needles in your arm. Also all evidence points to PD giving just as good dialysis as haemo.

So being a restless sleeper i guess nocturnal hemo is out.
So the CCPD is looking like what may best work for me. (hopefully)


Other then one being a machine and the other just hanging on a pole what is better in your opinion.

Hi there, I was once on hemo a few years back. I will soon be getting started on dialysis again in a few weeks. This time I am opting for p/d overnight with the cycler. Ask your hospital for dvd's about each form of dialysis. I believe the dvd's I saw a few months back were made by davita or baxter. There are lots of testimonials and an education part of the video. With P/D I am looking forward to providing my own care in my own home.

I was in PD training for a week.  The last day, they started me off on CCPD and I was only able to tolerate it for 4 nights and started CAPD (manual).
I don't want you to get discourage because everybody's experiences are different.  Some love CCPD (cycler) and have no problems.
I just happen to be the one that had a hard time with it. CAPD works best for me because I only do 2 exchanges.  I do 7.5% Icodextrin solution (purple cap) in the morning which is good for up to 18hrs (dwell time) and carry that fluid all day.  I do another exchange 2.5% Dextrose (green) before bedtime (6-8hrs dwell time).  You will learn the different types of solutions during PD training.

In my opinion, the "cycler" is not loud at all.  The only time it makes a sound is when it pumps fluid in and out.

I started nocturnal about a month and a half ago. I feel better and look better.
