Title: Vancomycin dosage for hemodialysis patients

The resident doctor at the ER suspects that my mother has a staph infection and gave here 1250 mg of Vancomycin initially. Is this a higher dose? I am reading online that the dosage is based off the weight and a lot of other factors, etc., My mother's weight is 64kg. If this dosage is too high, when should she get her next dose and how much is recommended? Thank you.

It's is as much about the level as the dose.  If they are drawing regular vanco levels, the dose will be adjusted appropriately (hopefully).  Also important to IV drop it over at least an hour to avoid risk of red man syndrome.
