Kt/V has been the primary measure of dialysis received for almost 40 years, but has come under significant criticism for not only its limitations but also in the way providers in some parts of the world manipulate its use at the expense of patient outcomes. The disturbing trend of decreasing time and increasing pump speed to maintain volume is being driven mainly from the US, which is seemingly ignoring advancements in understanding of appropriate dialysis dose.
We have just released a public calculator for measuring hemodialysis dose based on time only (known as Replaced Kidney Function or RKF). A kidney removes toxins 24/7 but a stage 5 patient may only get toxins removed for 5% of the time.
Example
- A fully functional kidney would rate of 100%.
- Standard HD would rate at between 5% and 7%
- Best Practice can reach 18% or higher.
Those achieving best practice in this rating are more likely to minimize post treatment complications, be fully employed, and on less medications. Its not about pump speed, its about percentage of time the body is free of toxin.
