ESRD Network: Suffolk County Regional Patient Education Meeting
May 18, 2011 from 1:00PM to 4:30PM at St. Catherine of Siena Nursing Home, 52 RTE 25A (Jericho Tpke & Rte. 25A), Smithtown, NY 11787
Description: This will be an afternoon meeting to present: Patient roles and responsibilities, sharing important health information and discussing many pertinent issues affecting dialysis patients today; such as: phosphorous management, vascular access, what is the ESRD Network, vocational rehabilitation, exercise and much more.
Attendance at the Annual Meeting is a requirement for registered facility Patient Representatives (PAC Reps). Please encourage PAC Representatives to attend and provide any assistance you can to help them to attend. Please encourage all of your facility PAC Reps and patients to attend this very important meeting.
If your facility does not have a patient representative, this is an excellent opportunity for patients to learn about what it takes to be a patient representative, who the IPRO ESRD Network is and how we can help.
Light refreshments will be provided.
